6) NYSC Certificate or Exemption Certificate

7) Sworn Affidavit/Age declaration documents for Npower volunteers that have changed their names

8) Birth certificate

9) Passport Photographs 

10) Physical verification information print out from nasims verification profile

11) Biometrics captured page printed from nasims verification profile

12) LASSRA Certificate (for Lagos volunteers) other states might have their certificate of residency

13) Utility bill, that is PHCN/prepaid bills, Waste Bill or other utility bill showing your residential address boldly.
